Sample Case of Unstageable Pressure Ulcer

Patient Background:
An 84-year-old female under hospice care for end-stage congestive heart failure (CHF) and advanced dementia presented with a worsening coccyx pressure ulcer. She is bed-bound and entirely dependent on caregivers for all activities of daily living (ADLs). Due to incontinence of both urine and stool, she requires frequent repositioning and comprehensive incontinence care.

Clinical Presentation:
The patient was referred for evaluation after the progressive deterioration of her pressure ulcer over several weeks despite ongoing treatment by another wound care provider. Her primary caregiver reported concerns about the wound becoming deeper, more unstable, and showing increased necrotic tissue. Previous interventions had not improved the wound condition. The caregiver was informed that such progression is typical in patients who are declining under hospice care, but they sought further assessment to ensure optimal comfort and wound management.
